A stepladder didn't fall on the council worker. He tripped over one that someone had left leaning against a racking unit in a storage room, at ankle height, at the end of a shift. Four broken ribs. Six weeks off work. An investigation that found every inspection record spotless.
The ladder had been checked that morning.
The Inspection Fetish
Irish workplaces have gotten reasonably good at the visible parts of ladder safety. Pre-use checks, weight ratings, competency training, tagging systems. The HSA's guidance is clear, prosecutions have happened, and most safety officers can recite the key points without looking at the ceiling.
What nobody talks about is what happens to the ladder after the job is done.
Storage is treated as self-evident. Of course you put the ladder away properly. That's not even a safety question, that's just tidiness. Except it isn't self-evident, and it isn't just tidiness, and the gap between inspection culture and storage culture is where a surprising number of people get hurt.
The council worker's case is not unusual. It is typical. Equipment that has been inspected, approved, and used correctly causes injury at the moment nobody is paying attention.
What "Proper Storage" Actually Means
Ask most workers where their stepladder should be stored and you'll get one of three answers: in the van, in the shed, or wherever there's space. None of those are wrong exactly, but none are complete either.
The HSA's guidance on work equipment, drawn from the Safety, Health and Welfare at Work (General Application) Regulations 2007, requires that equipment is stored in a way that does not create a hazard. That phrase does a lot of work. A ladder leaning against a wall at a 45-degree angle in a narrow corridor creates a hazard. A ladder stored flat on the floor near a doorway creates a hazard. A ladder hanging from two hooks at shoulder height in a dedicated storage bay does not.
Specific requirements worth building into your system:
Horizontal racking. Stepladders stored horizontally on wall-mounted racking, with the feet secured or the hinge locked, cannot fall and cannot be tripped over. This is the minimum standard for any fixed workplace. Vans need purpose-built ladder clamps, not bungee cords and wishful thinking.
Separation from traffic routes. Storage areas that double as walkways are where incidents happen. If your ladder lives anywhere that a person might reasonably walk, the storage method needs to account for that foot traffic.
Hinge locking. A folded stepladder stored upright with the hinge unlocked is a ladder waiting to open. It takes one nudge. Lock the hinge or store it horizontal.
Dedicated location. If the ladder doesn't have a specific home, it will end up wherever someone puts it last. That's how you get a ladder leaning against racking at ankle height at the end of a Friday shift.
The Last 10% Problem
There's a predictable pattern in how safety protocols degrade. The first 90% gets reinforced constantly: training, toolbox talks, near-miss reporting, inspections. The last 10% is assumed. And assumptions are where people get hurt.
Falls from height account for a significant proportion of serious workplace injuries in Ireland year after year. But the statistics rarely separate "fell from a ladder" from "fell over a stored ladder" or "was struck by a ladder that wasn't secured." The incident type matters for prevention. If your interventions are all aimed at use and none at storage, you're leaving a gap.
This is the same logic that applies to equipment in transit. A ladder that's fine on the job and fine in storage can still cause injury if the handoff between those two states isn't controlled. The moment between finishing work and securing the equipment is exactly where attention drops.
What a Storage Audit Should Cover
If you've never formally audited ladder storage at your workplace, here's what to look at.
Current storage locations. Walk the site. Find every ladder. Note where it actually is, not where it's supposed to be. If those are different, you have a system problem.
Method of storage. Is it horizontal or vertical? Secured or just resting? Is the hinge locked? Are the feet touching a surface that could cause slippage?
Proximity to walkways. Draw a one-metre buffer around any route people use. Anything stored within that buffer needs either a physical barrier or a different location.
Condition on return. Most pre-use inspection systems check the ladder before it goes out. Almost none check it when it comes back in. A rung that was fine at 7am may have been damaged during the job. If the ladder goes straight into storage without a post-use check, damaged equipment is sitting in the system waiting for the next use.
Accountability. Who is responsible for ensuring the ladder is stored correctly? If the answer is "everyone," the answer is no one. Name a person or a role.
The Investigation That Found Nothing Wrong
The council worker's case stings precisely because the paper trail was clean. Pre-use check completed. Weight rating appropriate. User trained. Inspection tag current.
None of that stopped him from tripping over it in a poorly lit storage room at 4:45pm on a Thursday.
An investigation that finds nothing wrong with the inspection record but ignores the storage conditions has only done half its job. The question isn't just "was the ladder safe to use?" It's "was the ladder managed safely across its entire working day?"
That includes the part nobody photographs for the safety file.
The Fix Is Not Complicated
A wall-mounted horizontal racking system for a standard stepladder costs under two hundred euro and takes an afternoon to install. A van ladder clamp costs forty. A laminated one-page storage procedure costs nothing.
The investment is not the obstacle. The obstacle is that storage has never been treated as a safety question. It gets bundled into housekeeping, which gets bundled into nobody's job description, which is why ladders end up leaning against racking at ankle height on a Thursday afternoon.
Treat storage as part of the equipment management cycle. Inspect it going out. Inspect it coming back. Store it to a defined standard. Name the person responsible. Check the storage location during your next site audit, not just the inspection records.
The broken ribs were preventable. Not by a better inspection system. By a hook on a wall and twenty seconds of attention at the end of a shift.
