A supervisor at a Shetland fish processing facility reached into running machinery to clear a blockage. The machine was not isolated. The injury was life-threatening. The fine that followed ran into six figures. None of it was inevitable.

This is not a rare story. It is a pattern. Fish processing, poultry lines, meat plants, dairy facilities across Ireland and the UK are producing near-identical incident reports year after year. The machinery changes. The species changes. The outcome does not.

The Blockage Problem Is a Culture Problem

Production lines in fish processing move fast. Blockages slow them down. The pressure to clear and continue is constant, often unspoken, and it shapes how workers respond when something jams.

The standard fix takes thirty seconds: reach in, pull out the obstruction, carry on. The problem is that thirty seconds is long enough for a conveyor, auger, or filleting blade to take a hand, a wrist, or worse.

What no one builds into that thirty-second calculation is the time it actually takes to properly isolate the machine. Switch off. Lock out. Tag out. Verify zero energy. That takes two minutes. Maybe three. In a plant running on tight margins and tighter schedules, those two minutes feel like an eternity. So the shortcut happens. And eventually, the shortcut finds you.

What Lockout-Tagout Actually Requires

Lockout-tagout (LOTO) is the formal procedure for ensuring machinery cannot be energised while someone is working on or inside it. The principle is simple. The execution is where plants fall apart.

A proper LOTO procedure covers six steps. Identify all energy sources for the machine, including electrical, hydraulic, pneumatic, and gravity-fed systems. Notify affected workers. Shut down using the established procedure. Isolate every energy source with a physical device, a lockout hasp, a valve lockout, a circuit breaker lock. Apply your personal padlock and tag. Verify the machine is at zero energy state before touching anything inside it.

That last step gets skipped constantly. Workers switch off the main power and assume the machine is safe. Residual hydraulic pressure. A capacitor holding charge. A gravity-loaded component. Any of these can move after the power goes off. Verification is not optional. It is the whole point.

In the Shetland incident, investigators found no formal LOTO procedure existed for blockage clearing. The supervisor was not following a shortcut. He was following the only process available to him, which was no process at all.

Where Irish Food Plants Are Falling Short

Irish food processing facilities operate under the Safety, Health and Welfare at Work (General Application) Regulations 2007, with specific machinery provisions under Part 2. The Health and Safety Authority has the power to inspect, issue improvement notices, and prosecute. It does all three.

The gap is not usually awareness of LOTO as a concept. Most plant managers know the term. The gap is in implementation. Specifically in three areas.

Written procedures that match actual equipment. Generic LOTO documents downloaded from a trade body do not count. A procedure needs to name the machine, identify every energy source on that machine, and specify the exact isolation devices used. If your procedure says "isolate at the main panel" for a machine with four separate energy inputs, your procedure is wrong.

Personal locks, not shared locks. A shared padlock defeats the purpose. If one key opens the lock, anyone can remove it, including someone who does not know you are still inside the machine. Every worker who may be exposed to the hazard gets their own lock. Their own key. Non-transferable.

Training that covers blockage clearing specifically. General machinery induction is not enough. Repetitive injuries in food manufacturing follow the same pattern: plants train workers on startup and shutdown but treat blockage clearing as self-evident. It is not. It is the highest-risk routine task on most lines, and it needs its own procedure.

The Supervisor Factor

The Shetland case involved a supervisor, not a new starter. That matters. It strips away the training-gap defence that employers often reach for first.

Supervisors clear blockages because they are physically present when it happens, because they feel responsible for keeping the line moving, and because asking someone else to do it while they stand back feels like poor leadership. That instinct is understandable. It is also what puts experienced workers in hospital.

The fix is not to remove supervisors from the equation. It is to make the correct procedure so embedded that it is the only option anyone considers. When clearing a blockage without isolating first is genuinely inconceivable to every person on the floor, LOTO is working. Until then, it is a document in a folder.

What a Fine Actually Costs

The financial penalty in the Shetland case was significant. But the total cost to the company was not the fine. It was the fine, plus investigation costs, plus legal fees, plus insurance uplift, plus the productivity loss during the investigation period, plus the cost of the remediation work the improvement notice required, plus the reputational damage in a sector where contracts depend on audit ratings.

Machinery safety failures are costing companies millions and workers their limbs across multiple industries. Food processing is not exempt, and the HSA's enforcement trend is moving toward prosecution rather than advisory notices when serious injuries are involved.

A LOTO programme costs money to implement properly. Lockout devices, hasps, tags, and padlocks for a medium-sized processing line run to a few hundred euro. Training takes time. Writing procedures takes longer. None of it approaches the cost of a single serious incident.

The Confined Space Overlap

Many blockage-clearing tasks in fish processing happen inside or partially inside equipment: hoppers, tanks, screw conveyors, chilling units. Once a worker enters a restricted space within a machine to clear an obstruction, this is no longer just a LOTO issue. It becomes a confined space entry.

Confined space entry requires a different and additional layer of controls: atmospheric testing, a rescue plan, a standby person, and formal permit-to-work. Fish processing plants frequently handle the machinery isolation but skip the confined space requirements because the space feels familiar and the entry feels brief. Regulatory inspectors do not accept that reasoning. Neither do courts.

If any part of your blockage-clearing task involves entering or reaching significantly into an enclosed area of machinery, your LOTO procedure needs to cross-reference your confined space permit system. If you do not have a confined space permit system, that is the starting point.

Fix the System, Not the Blame

After the Shetland incident, the investigation did not find a reckless supervisor. It found a plant where the pressure to maintain throughput was stronger than the procedure to manage risk, where LOTO existed in name but not in practice, and where nobody had formally considered what to do when a blockage occurred on that specific machine.

That is a system failure. It sits with management, not with the person who got hurt.

If your plant's honest answer to "what do workers do when that conveyor jams?" is "they use their judgement," you have not managed the risk. You have delegated it downward to the person with the least power and the most exposure.

Write the procedure. Buy the locks. Train the team. Audit it quarterly. The alternative has a very well-documented cost.
