The van door closes. The lads head to site. Nobody checks the grinder that has been rattling since February.

That is how three fatal incidents in recent months started. Not with dramatic failures. Not with equipment that screamed danger. With ordinary tools that had been quietly deteriorating for months while everyone assumed someone else had checked them.

The Pattern Behind the Incidents

Each of the three incidents involved equipment that was in regular daily use. A power tool with a damaged guard. A lifting accessory with concealed corrosion on the load-bearing point. A pressure vessel that had not been formally inspected in over two years. None of them were flagged. All three had been used that morning without issue, right up until the moment they weren't.

The investigation findings in each case pointed to the same failure: no formal inspection regime, no documented pre-use checks, and a site culture where deteriorating equipment was normalised rather than removed from service. Machinery incidents that result in criminal negligence prosecutions almost always contain this same thread. The equipment was known to be worn. Someone made a judgement call that it was fine for now. It was not fine.

Under the Safety, Health and Welfare at Work (General Application) Regulations 2007, work equipment must be maintained in an efficient state, in efficient working order, and in good repair. That is not aspirational language. It is a legal standard. If your equipment falls below it and someone gets hurt, you own the outcome.

What the Warning Signs Actually Look Like

Inspectors and investigators tend to find the same physical indicators on equipment involved in serious incidents. These are not edge cases. They appear on sites across Ireland every week.

Guards and protective covers: Cracked, missing, or secured with cable ties instead of the original fixings. A guard held on by a zip tie is not a guard. It is a sign that the proper guard was damaged, nobody ordered a replacement, and the workaround became permanent.

Cables and power leads: Repaired with electrical tape rather than replaced. Exposed conductors near the plug or at the tool end. Cables that have been run over by site vehicles and flattened. Any of these on a 110V or 240V tool are removal-from-service issues, not monitor-and-review situations.

Lifting accessories: Chains, slings, shackles, and hooks that show visible deformation, rust at load-bearing points, or wear marks from contact with sharp edges. The Safe Working Load marking being worn or illegible. Shackle pins that are not moused or secured. These items fail without warning and they fail completely.

Angle grinders: Discs that are chipped, cracked, or running off-centre. Vibration that has developed recently. The guard adjusted or removed to reach awkward angles and never put back. Angle grinders cause some of the most severe hand and face injuries on site, and most of them involve a disc that had visible damage before the incident.

Pressure equipment: No visible service record. Hose connections showing perishing or cracking near the fittings. Relief valves that have never been tested. Vessels that are more than twelve months past their last formal inspection.

Why Nobody Catches It

The honest answer is that pre-use checks are treated as a formality rather than a genuine assessment. Workers tick boxes because they have been told to tick boxes. Nobody demonstrated what they are actually looking for. Nobody ever removed equipment from service as a result of a check, so the check has no perceived consequence either way.

There is also a normalisation problem. Equipment that degrades slowly does not trigger alarm. A grinder that vibrated a little more than usual last month vibrates a bit more than that this month. Everyone adjusts. The baseline shifts. By the time the tool is genuinely dangerous, it feels normal.

Supervisors are not immune to this. The tendency is to weigh the cost of taking equipment out of service against the cost of getting the job finished. That calculation ignores the third figure in the equation, which is the cost when the equipment kills someone. That figure runs to prosecutions, fines north of €100,000, civil claims, and the kind of reputational damage that closes companies. Unguarded machinery prosecutions consistently follow this pattern and the fines reflect it.

How to Audit Your Own Gear

This does not require a consultant. It requires someone with authority to remove equipment from service and a structured approach to looking at what is actually in the van and on the site.

Start with inventory. You cannot inspect what you have not listed. Every item of work equipment should be on a register with a reference number, purchase or service date, and inspection history. If you cannot produce this for an HSA inspector, you are already in difficulty.

Check inspection currency. Lifting equipment requires thorough examination every six months for equipment used to lift people, and twelve months for other lifting equipment, under the General Application Regulations. Pressure vessels have their own examination schedules. Check the last formal inspection date for every item on your register. If it has passed, the equipment comes out of service until it is examined.

Physical inspection per item. Guards intact and properly secured. No improvised repairs. Cables and hoses in original condition or formally replaced. Labels and markings legible. Moving parts operating without abnormal noise or vibration. Anything that fails this check is tagged out and removed.

Tag-out system. If you do not have physical tags for defective equipment, get them. A verbal instruction to stop using something is forgotten by the next shift. A bright red tag on the tool itself is not.

Sign-off culture. Pre-use checks only work if the person doing them knows they will be asked about them. Spot-check the records. Ask workers what they are looking for. If the answer is vague, the checks are not working.

The Turn

None of this is technically complex. The equipment that killed three workers in recent months was not exotic machinery. It was the same gear that is in vans across the country right now. The difference is not what the equipment looked like. It is whether anyone with authority actually looked at it.

Audit your gear this week. Not next month. This week.
